New Patients

New patients, prior to your first massage appointment, please print out the patient intake forms, complete the forms at your convenience, then bring them with you to your appointment.

Please:

• be prepared to pay at the time of your massage.

If your massage treatment is specifically for a work or auto accident related injury, please bring the following information with you:

• date of injury/accident,
• claim number,
• name and phone number of claim representative, and
• billing address (if being covered by auto insurance).

Your First Appointment

You will be greeted by Mike Gibbons, LMT.  Mike will review your paperwork and discuss your health concerns with you.  Massage therapy treatments will always be tailored to your needs and comfort level.
